Instructions for completing Form 30

The report on the activities of the health care institution of the system of the Ministry of Health and Social Development of the Russian Federation (Ministry of Health and Social Development of Russia) is drawn up by all health care institutions serving patients in the assigned territory.

The report is submitted for the year as a whole for the institution, local government in the field of health care on January 10 of the following year.

The local self-government body in the field of health care shall submit a summary report for the district and the report of each subordinate institution to the health administration body of the constituent entity of the Russian Federation by February 10 of the following reporting year, within the time frame established by the health administration body of the constituent entity of the Russian Federation.

The consolidated report on the constituent entity of the Russian Federation for the reporting year, signed by the head of the healthcare management body in 2 copies on paper and electronic media, is submitted to the Ministry of Health and Social Development of Russia until March 5 of the following year after the reporting year, within the time frame established by the Ministry of Health and Social Development of the Russian Federation, territorial to the Rosstat body in the constituent entity of the Russian Federation at the address set by it and the TF OMS of the constituent entity of the Russian Federation by March 20 of the next reporting year.

Institutions that have a hospital and a polyclinic (or several polyclinics), including institutions that only consult patients, draw up a report for the entire institution as a whole.

A hospital that does not have a polyclinic unit and does not provide outpatient care does not fill out Section II, and unified polyclinic (independent) institutions (polyclinics, outpatient clinics, dispensaries without hospitals) do not fill out Section III.

All healthcare facilities complete Title I Facility States. All information in f. N 30 are provided in absolute numbers.

The passport part of the title page is filled out clearly in accordance with the name of the line (the type of institution is indicated in accordance with the nomenclature of institutions approved by the order of the Ministry of Health and Social Development of Russia, its postal address). Medical and sanitary units indicate the name of the serviced enterprise in whose jurisdiction they are, clinics of universities and research institutes, clinical hospitals indicate the full name of a medical or research institute, clinical

Rosstat has updated the federal statistical monitoring form No. 30 "Information about a medical organization." This form, according to the order of Rosstat dated August 3, 2018 No. 483 "On the approval of statistical tools for the organization by the Ministry of Health of the Russian Federation of federal statistical observation in the field of health protection", begins to operate with the report for 2018.

Deadlines for submitting a Form 30 report for medical organizations

This form of statistical observation No. 30 must be submitted by all medical organizations that are included in the nomenclature of medical organizations (order of the Ministry of Health of Russia dated 06.08.2013 N529n). For clinics of higher education and research institutes, they must also complete this form and generate a separate summary form.

Terms of submission of reporting in form No. 30 for self-government bodies

The form is submitted for the year by the medical organization to the local government on January 20 of the year following the reporting period. Local authorities must submit a report on each subordinate medical organization to the executive authority of the constituent entity of the Russian Federation following the reporting period.

Terms of submission of the summary report for the constituent entities of the Russian Federation

The consolidated report on a constituent entity of the Russian Federation for the reporting year in 2 copies on paper and electronic media is submitted to the Ministry of Health of Russia within the time frame established by the Ministry of Health of Russia until March 5, and to the territorial body of Rosstat in the constituent entity of the Russian Federation at the address specified by it - on March 25.

Filling out the address part of the form No. 30

The address part of the form indicates the full name of the reporting organization in accordance with the constituent documents, registered in the prescribed manner, and then in brackets - the short name.

On the line "Postal address" the name of the constituent entity of the Russian Federation, legal address with postal code shall be indicated; if the actual address does not coincide with the legal address, then the actual (postal) address is also indicated.

Filling out form No. 30 for an organization with separate divisions

If a legal entity has separate subdivisions *, this form is filled in both for each separate subdivision and for the legal entity without these separate subdivisions.

What medical organization is legal. a person with separate subdivisions?

Separate subdivision of an organization - any subdivision geographically separate from it, at the location of which stationary workplaces are equipped. Recognition of a separate division of an organization as such is carried out regardless of whether its creation is reflected or not reflected in the constituent or other organizational and administrative documents of the organization, and from the powers that are vested in the said division. In this case, a workplace is considered stationary if it is created for a period of more than one month (clause 2 of article 11 of the Tax Code of the Russian Federation).

Where to submit reports?

The completed form is submitted by a legal entity to local government bodies exercising powers in the field of health protection at the location of the corresponding separate division (for a separate division) and at the location of the legal entity (without separate divisions). In the event that a legal entity (its separate subdivision) does not carry out activities at the place of its location, the form is provided at the place of its actual implementation of activities.

Who should be delegated filling out the report on Form 30?

The head of the legal entity appoints officials authorized to provide primary statistical data on behalf of the legal entity. In case of delegation of authority to provide statistical reporting on behalf of the legal entity to a separate subdivision, a separate subdivision in the code part of the form indicates the OKPO code (for a branch) or an identification number (for a separate subdivision that does not have the status of a branch), which is established by the territorial body of Rosstat at the location of the separate subdivision.

Reporting for multidisciplinary healthcare facilities

Multidisciplinary medical organizations providing various types of medical care, including departments providing consultative reception, compile a report for the entire organization.

Which tables are optional

  • Medical organizations fill in and print only those tables that relate to the activities of these medical organizations.
  • Medical organizations that do not provide medical care on an outpatient basis do not complete Section III.
  • Medical organizations that do not provide medical care in inpatient conditions do not fill in Section IV.
  • All medical organizations, without exception, must fill out the tables: 1000, 1001, 1100, 7000, 8000 forms.
  • All information in the form is provided in whole numbers.
  • The definitions and explanations provided in these Guidelines on the procedure for generating indicators are intended solely for the purpose of filling out this form.
