In the Russian Federation, it is planned to update the nomenclature of medical posts. The document must go through all the necessary stages of approval before the end of the current year.

According to Anastasia Gazheva, director general of the Association of Specialists with Higher Nursing Education, some medical specialties will be excluded from the nomenclature. As she said at the meeting “Implementation of pilot projects and regulations for the provision of medical care in the regions”, it is with this reduction in mind that new standards of professional activity will be developed.

In particular, the new standards will determine the changing nature of the work of nurses. According to the innovation, the nurse will deal with 15-17 patients instead of the current 60, but will take on all the work related to patient care.

Nursing positions in the medical rehabilitation industry will also be combined, which should help to cope with the shortage of personnel. According to Gazheva, the deficit was caused not so much by a lack of personnel, but by an artificial layering of posts. In some constituent entities of the Federation, such a change is already in effect; now it will be introduced throughout Russia.

It is planned to introduce new posts designed specifically for nurses with higher nursing education. In addition, the currently existing positions of “procedural nurse”, “fasting nurse”, “dressing nurse” will be combined into one “clinical nurse”.

One of the changes will be the separation of the specifics of the work of the family doctor and general practitioner. The family doctor will work with both adults and children, and only adult patients will remain with the general practitioner.

Already this year, the appearance of new professional standards for doctors of various specialties is planned. The adoption of new standards is also expected for nurses, however, while these standards are still under development.

Developed and published professional standards for the nurse and medical registrar. According to these standards, a medical registrar may not have a medical education, but will be required to undergo special training.

For employees in senior positions, a new standard, “Specialist in the field of healthcare organization”, has also been developed.

In 2019, for the first time, baccalaureates will be accredited, which are currently being taught by twenty-five higher medical schools. A professional standard has also been issued for graduates, according to which bachelors will be able to work in medical centers of preschool and school institutions.

Nomenclature of posts

It is fixed by the order of the Ministry of Health of Russia dated 12.20.2012 No. 1183n (Order No. 1183n). In comparison with the previous listing, the modification is quite serious. The main change is the emerging definition of "job title." This definition replaced the previous definition of “personnel position” (according to the Federal Law of 21.11.2011 No. 323-ФЗ “On the Basics of Protecting Citizens' Health in the Russian Federation”). Several medical posts related to such areas of medicine as preventive, palliative, rehabilitation are added. An osteopath post has also been added to the post register.

The medical position of a doctor of restorative medicine has been removed from the registry.

What happens when a staffing table is drawn up, guided by the norms of orders of the Ministry of Health of Russia, according to the procedures for providing medical care? With this rather frequent practice, the nomenclature of posts is not respected. This is due to the fact that job titles in orders often do not coincide with the current nomenclature (table 1).

Nomenclature of medical specialties

This registry is approved by the following orders:

  • by order of the Ministry of Health and Social Development of Russia dated 16.04.2008 No. 176n “On the nomenclature of specialties of specialists with secondary medical and pharmaceutical education in the field of healthcare of the Russian Federation” (hereinafter - Order No. 176n);
  • by order of the Ministry of Health of Russia dated 07.10.2015 No. 700n “On the nomenclature of specialties of specialists with higher medical and pharmaceutical education” (hereinafter - Order No. 700n).

Conventionally breaking into two parts on the basis of the education received, the nomenclature of medical specialties includes 35 specialties for secondary education and 94 specialties for higher education.

The medical community had hopes for order number 700n. It was expected that he would eliminate the lack of unity between the nomenclature of specialties and medical posts. With the publication of the order, it became clear that this did not happen.

Any change in such a nomenclature as medical posts must be accompanied by a change in the nomenclature of specialties and vice versa. For example, the introduction of the medical posts of a doctor for medical prevention and a doctor for palliative care in 2012 should be accompanied by a footnote or note: “in connection with the publication of order No. 1183n, amend Order No. 2102 ...”. This has not been done. As a result, the last three years there has been a mismatch of nomenclatures.

Order No. 700n differs from the previous order of the Ministry of Health and Social Development of Russia dated April 23, 2009 No. 210 in the construction architecture and the number of specialties.

Previously, specialties were indicated according to the hierarchical principle:

  • specialty, which was received at the university;
  • main specialty;
  • specialty, which requires additional training.

Order No. 700n no longer provides for this differentiation.

Order No. 700n ruled out specialties: nutrition, laboratory mycology.

Qualification requirements

To draw parallels between the nomenclature of medical specialties and medical posts, it is reasonable to use the orders of the Ministry of Health of Russia:

  • dated 08.10.2015 No. 707n “On approval of the Qualification requirements for medical and pharmaceutical workers with higher education in the area of \u200b\u200btraining“ Health and Medical Sciences ”” (hereinafter - Order No. 707n);
  • dated 02.10.2016 No. 83n “On the approval of Qualification requirements for medical and pharmaceutical workers with secondary medical and pharmaceutical education”.

The listed orders, although not fully, can clarify the correlation of nomenclatures. After all, these documents for each specialty indicate data on the level of vocational education, on education received by the employee additionally, on the position.

The names of specialties in these documents almost completely correspond to the current orders on the nomenclatures of specialties, i.e., Order No. 176n and Order No. 700n. An exception - laboratory mycology is not in the list of specialties. At the same time, in Order No. 707n, as an additional professional education in the specialty "Bacteriology", laboratory mycology is present as a professional retraining. So, laboratory mycology is recognized as a specialty.

Comparison of the nomenclature of posts and qualification requirements

If we compare the names of posts in Order No. 707n and Order No. 83n with the nomenclature of posts, again inconsistencies are found.

For these positions, labor standards were developed (approved by orders of the Ministry of Health of Russia). The availability of labor standards can serve as the basis for the introduction of these posts in the staffing table.

We remind you that according to the Labor Code of the Russian Federation, a professional standard is a characteristic of the qualifications necessary for an employee to carry out a certain type of professional activity, including performing a specific labor function.

On the basis of what professional standards are applied?

Professional standards are developed and applied in accordance with Article 195.2 and Article 195.3 of the Labor Code of the Russian Federation.

Mandatory application of professional standards?

In accordance with Art. 195.3 of the Labor Code of the Russian Federation established that if a The Labor Code of the Russian Federation, other federal laws, other regulatory legal acts of the Russian Federation qualification requirements establishednecessary for the employee to perform a specific labor function, professional standards regarding these requirements are mandatory for employers.

Note that according to Art. 57 of the Labor Code of the Russian Federation, if the employee’s labor function (job position in accordance with the staffing list, profession, specialty with qualifications; a specific type of work assigned to the employee), that is, work performed on certain posts, professions, specialties is related to the provision of compensation and privileges or the existence of restrictions, the name of these posts, professions or specialties and qualification requirements for them must correspond to the names and requirements specified in the qualification handbooks or relevant provisions of professional standards.

For state and municipal medical organizations in accordance with the Decree of the Government of the Russian Federation dated June 27, 2016 No. 584, professional standards in terms of qualification requirements are applied in stages based on the plans for organizing the application of professional standards approved by the indicated organizations taking into account the opinions of the representative bodies of employees, :

a) a list of professional standards to be applied;

b) information on the need for vocational education, vocational training and (or) additional vocational education of employees, obtained on the basis of the analysis of qualification requirements contained in professional standards, and on the implementation of relevant education and training activities in the established manner;

c) the stages of applying professional standards;

d) a list of local regulations and other documents of organizations, including on certification, certification and other forms of assessment of qualifications of employees subject to change in connection with the provisions of professional standards to be applied.

The Decree of the Government of the Russian Federation established that the implementation of the measures of the plans should be completed no later than January 1, 2020.

The Ministry of Health has approved a regulation on the accreditation of medical workers.

It was decided to create on the basis of North-West State Medical University. I.I. Mechnikov Federal Methodological Center for Osteopathy.

The stages and timing of the transition of medical and pharmaceutical workers to the accreditation procedure are defined.

The Ministry of Health has approved vocational retraining and continuing education programs in the specialty Osteopathy.

Osteopathy is included in the nomenclature of specialties of higher medical education.

Qualification requirements for osteopathic doctors approved.

Dmitry Evgenievich Mokhov was appointed the chief freelance specialist of the Ministry of Health, specializing in Osteopathy.

The main freelance specialists of the Ministry of Health were appointed with a degree in Osteopathy in federal districts.

Specialty 08/31/52 "Osteopathy" is included in the list of specialties for training highly qualified personnel on residency programs.

The nomenclature of medical services includes initial and repeated admission (examination, consultation) of an osteopathic doctor.

The post "osteopathic doctor" is included in the nomenclature of posts of medical and pharmaceutical workers.
