In the Russian Federation, it is planned to update the nomenclature of medical positions. The document must go through all the necessary stages of approval before the end of the current year.

According to Anastasia Gazheva, General Director of the Association of Specialists with Higher Nursing Education, some medical specialties will be excluded from the nomenclature. As she said at the meeting "Introduction of pilot projects and regulations for the provision of medical care in the regions", it is with this reduction in mind that new standards of professional activity will be developed.

In particular, the new standards will change the nature of nursing work. According to the innovation nurse will deal with 15-17 patients instead of the current 60, but will take over all the work related to patient care.

Nursing positions in the field of medical rehabilitation will also be merged, which should help to cope with the resulting shortage of personnel. According to Gazheva, the deficit was due not so much to a shortage of personnel, but to an artificial stratification of positions. In some subjects of the Federation, such a change is already in effect, now it will be introduced throughout Russia.

It is planned to introduce new positions designed specifically for nurses with higher nursing education. In addition, the current positions of “procedure nurse”, “post nurse”, “dressing nurse” will be merged into one “clinical nurse”.

One of the changes will be the separation of the specifics of the work of a family doctor and a general practitioner. The family doctor will work with both adults and children, while only adult patients will remain in charge of the general practitioner.

Already this year, new professional standards for doctors of various specialties are planned to appear. The adoption of new standards for paramedical personnel is also expected, however, while these standards are under development.

Developed and published professional standards for nurses and medical registrars. According to these standards, medical registrar may not have a medical education, but will be required to undergo special training.

For employees who leadership positions, also developed new standard"Specialist in the field of healthcare organization."

In 2019, for the first time, bachelors will be accredited, who are currently taught by twenty-five higher medical educational institutions. A professional standard has also been issued for graduates, according to which bachelors will be able to work in first-aid posts of preschool and school institutions.

How do these requirements relate to the nomenclature of positions

Unfortunately, normatively, such concepts as profession, specialty and position in health care, being used quite often, are not actually defined. It is proposed to use the following definitions.

Profession - type labor activity responsible for specific training. As a rule, it is a source of existence.

A specialty characterizes a separate branch of skill, profession, or basic qualification.

Position - the primary structural unit of the staffing table, its job responsibilities. With regard to medical positions, there is always a documented, clearly defined scope of work.

Nomenclature of positions

It is fixed by the order of the Ministry of Health of Russia dated December 20, 2012 No. 1183n (Order No. 1183n). In comparison with the previous enumeration, the modification is quite serious. The main change is the new definition of “employee positions”. This definition replaced the previous definition of “staff position” (according to the Federal Law of November 21, 2011 No. 323-FZ “On the Fundamentals of Protecting the Health of Citizens in the Russian Federation”). Several medical positions related to such areas of medicine as preventive, palliative, and rehabilitation have been added. The position of an osteopath has also been added to the register of positions.

The medical position of a doctor of restorative medicine has been removed from the register.

What happens when the staffing table is drawn up, guided by the standards of orders of the Ministry of Health of Russia, according to the procedures for providing medical care? With this rather frequent practice, the nomenclature of positions is not respected. This is due to the fact that the names of positions in orders often do not coincide with the current nomenclature (table 1).

Nomenclature of medical specialties

This register is approved by the following orders:

  • by order of the Ministry of Health and Social Development of Russia dated April 16, 2008 No. 176n “On the nomenclature of specialties for specialists with secondary medical and pharmaceutical education in the field of healthcare of the Russian Federation” (hereinafter - Order No. 176n);
  • by order of the Ministry of Health of Russia dated October 7, 2015 No. 700n “On the nomenclature of specialties for specialists with higher medical and pharmaceutical education” (hereinafter - Order No. 700n).

Conditionally divided into two parts on the basis of the education received, the nomenclature of medical specialties includes 35 specialties for secondary education, 94 specialties for higher education.

The medical community pinned its hopes on Order No. 700n. It was expected that he would eliminate the lack of unity between the nomenclature of specialties and medical positions. With the publication of the order, it became clear that this did not happen.

Any change in such a nomenclature as medical positions must be accompanied by a change in the nomenclature of specialties and vice versa. For example, the introduction of medical positions of a doctor for medical prevention and a doctor for palliative care in 2012 should have been accompanied by a footnote or note: “in connection with the publication of order No. 1183n, amend order No. 2102 ...”. This was not done. As a result, there has been a discrepancy between the nomenclatures for the last three years.

Order No. 700n differs from the previous order of the Ministry of Health and Social Development of Russia dated April 23, 2009 No. 210 in the architecture of construction and the number of specialties.

Previously, specialties were indicated according to a hierarchical principle:

  • the specialty that was obtained at the university;
  • main specialty;
  • a specialty that requires additional training.

Order No. 700n no longer provides for this differentiation.

Order No. 700n excluded specialties: dietetics, laboratory mycology.

Qualifications

To draw parallels between the nomenclature of medical specialties and medical positions, it is reasonable to use the orders of the Ministry of Health of Russia:

  • No. 707n dated 08.10.2015 “On Approval of Qualification Requirements for Medical and Pharmaceutical Workers with higher education in the direction of training "Health care and medical sciences" (hereinafter - Order No. 707n);
  • No. 83n dated February 10, 2016 “On Approval of Qualification Requirements for Medical and Pharmaceutical Workers with Secondary Medical and Pharmaceutical Education”.

The listed orders, although not in full, but can clarify the correlation of nomenclatures. After all, these documents for each specialty indicate data on the level of vocational education, on the education received by the employee in addition, on the position.

The names of specialties in these documents almost completely correspond to the current orders on the nomenclatures of specialties, i.e. Order No. 176n and Order No. 700n. Exception - laboratory mycology is not in the list of specialties. At the same time, in Order No. 707n, as an additional vocational education in the specialty "Bacteriology", laboratory mycology is present as professional retraining. This means that laboratory mycology is recognized as a specialty.

Comparison of the nomenclature of positions and qualification requirements

If we compare the names of positions in Order No. 707n and Order No. 83n with the nomenclature of positions, inconsistencies are again found.

Labor standards were developed for these positions (approved by orders of the Ministry of Health of Russia). The presence of labor standards can serve as the basis for the introduction of these positions in the staffing table.

We remind you that according to the Labor Code of the Russian Federation, a professional standard is a characteristic of the qualifications necessary for an employee to carry out a certain type of professional activity, including the performance of a certain labor function.

On what basis are professional standards applied?

Professional standards are developed and applied in accordance with Article 195.2 and Article 195.3 Labor Code Russian Federation.

Mandatory application of professional standards?

In accordance with Art. 195.3 of the Labor Code of the Russian Federation establishes that if Labor Code of the Russian Federation, other federal laws, other regulatory legal acts of the Russian Federation established qualification requirements necessary for the employee to perform a certain labor function, professional standards in terms of these requirements are mandatory for employers to apply.

We note that according to Art. 57 of the Labor Code of the Russian Federation, if labor function employee (work according to the position in accordance with staffing, professions, specialties indicating qualifications; the specific type of work assigned to the employee), that is, the work performed in certain positions, professions, specialties is associated with the provision of compensation and benefits or the presence of restrictions, then the name of these positions, professions or specialties and qualification requirements they must comply with the names and requirements specified in the qualification reference books or the relevant provisions of professional standards.

For state and municipal medical organizations in accordance with Decree of the Government of the Russian Federation of June 27, 2016 No. 584, professional standards in terms of qualification requirements are applied in stages based on plans approved by the said organizations, taking into account the opinions of the representative bodies of employees, plans for organizing the application of professional standards, containing, among other things:

a) a list of professional standards to be applied;

b) information on the need for vocational education, vocational training and (or) additional vocational education of employees, obtained on the basis of an analysis of the qualification requirements contained in professional standards, and on the implementation of relevant education and training activities in the prescribed manner;

c) stages of application of professional standards;

d) a list of local regulations and other documents of organizations, including those on attestation, certification and other forms of assessing the qualifications of employees, subject to change in connection with the provisions of professional standards to be applied.

The Decree of the Government of the Russian Federation established that the implementation of the activities of the plans must be completed no later than January 1, 2020.

Are occupational health standards mandatory?

Since the requirements for the qualification of medical workers are established:

In Russia, the Procedure for the provision of medical care in the field of osteopathy began to operate. It was approved by the order of the Ministry of Health No. 21n dated January 19, 2018. The document defines the rules for organizing osteopathic care, its types, forms and conditions of provision.

The Ministry of Health approved a new range of medical services. The corresponding departmental order reflects all the concepts of osteopathy included in the clinical guidelines.

The Ministry of Health has changed the qualification requirements for osteopathic doctors, supplementing them with the specialty "Dentistry".

The Ministry of Education and Science has approved the introduction of a specialty in osteopathy in medical schools - osteopathy has become one of the basic medical specialties along with general medicine, pediatrics and dentistry.

By the Decree of the Government of the Russian Federation, osteopathy is included in the list of works (services) that make up medical activity and subject to licensing.

The Ministry of Health approved the regulation on the accreditation of medical workers.

A decision was made to create on the basis of the North-Western State Medical University named after I.I. I.I. Mechnikov Federal Methodological Center for Osteopathy.

The stages and terms of the transition of medical and pharmaceutical workers to the accreditation procedure have been determined.

The main freelance specialists of the Ministry of Health in the specialty "Osteopathy" were appointed in the federal districts.

Specialty 31.08.52 "Osteopathy" is included in the list of specialties for the training of highly qualified personnel in residency programs.

The range of medical services includes primary and repeated appointments (examination, consultation) with an osteopath.

The position of "doctor-osteopath" is included in the nomenclature of positions of medical and pharmaceutical workers.
